REFERRALS

For Referring Physicians

  • Referrals through OCEAN e-referral are preferred.

  • Ensure your OHIP billing number and contact information for the patient are included. 

  • Please include a patient email whenever possible. We are working towards electronic communication to improve the care for our patients. 

New referrals must include:

  1. Differential/Suspected diagnosis.

  2. Any previous dermatology notes.

  3. Any first-line treatments tried to date.

open to new Referrals

We are open to all new referrals, with the following exceptions:

  • Warts

  • Cosmetic lesion removal/benign lesions: seborrheic keratoses/skin tags/sebaceous cysts

  • “Skin checks” – full skin exams without a specific lesion of concern or significant skin cancer risk factors (as per the Cancer Care Ontario skincare pathways guidelines).
